	Inform our staff the reason for making the appointment so appropriate time will be provided
	Download New Patient Forms
	Complete forms and return to office for appointment
	Provide legible past dental records and or x-rays were applicable*
	*radiographs, dental records and models are diagnostic tools and in the event they are not current and or not legible or "clear" new diagnostic tools will be necessary to provide a thorough exam.
